fixes to assessment tool:

* added numbering offset to the question list

* shuffle questions and their options

* matching pairs' <select> is also shuffled now

* fixed assessment of ordering question type

* improved short answer and numerical jsps

* short answer now supports wildcard "*"

* "Notify teacher after student completes attempt" implemented
